The NGO has conducted several camp on awareness of chronic disease management, Prevention of STD, since 2015.
Chronic diseases are long-term conditions that usually progress slowly over time. This includes conditions such as heart disease, diabetes, chronic kidney disease, Thyroid disease, Hypertenson, asthma and chronic obstructive pulmonary disease (COPD), depression, and arthritis, among many others.
Many of the most common chronic diseases can be linked to patient lifestyle choices, missed opportunities to deliver preventative care, and a health system that does not prioritize well-care over sick-care due to financial pressures.
Our health care system needs to align incentives to encourage payers, providers, employers, and individuals to better prevent, detect, treat, and manage chronic diseases — both physical and mental — before they become acute, costly problems
The report states, urging providers to take advantage of new value-based reimbursement models that highlight the necessity to deliver robust chronic disease management and complete preventative care.
Chronic Disease Management (CDM) is ongoing care and support to assist individuals impacted by a chronic health condition with the medical care, knowledge, skills and resources they need to better manage on a day to day basis. This may include regular visits and support from your family physician, other primary care provider, community-based programs or referrals to specialist programs and services.
Awareness Camp on Thyroid Disease Management-
The NGO repeatedly organized thyroid detection since last two and hulf year camp for the people from different villages where almost 900 patients were detected with thyroid disorder and almost 550 patient are regularly following up with us the list of beneficiary may be produced on needed.
Awareness cum Diabetes disease detection camp.-
The NGO has organized awareness cum diabetes disease detection camp where patient were make aware about to Keep their blood glucose levels as near to normal as possible by balancing food intake with medication and activity, maintain blood cholesterol and triglyceride (lipid) levels as near the normal ranges as possible, Control blood pressure and tips to decrease or possibly prevent the development of diabetes-related health problems to hold the keys to managing diabetes by:
1. Planning what you eat and following a balanced meal plan
2. Exercising regularly
3. Taking medication, if prescribed, and closely following the guidelines on how and when to take it
4. Monitoring blood glucose and blood pressure levels at home.
Almost 1100 people were detected with diabetes mellitus and patient is regularly following up by us.
Awareness Camp on Vaccination-
The Member of the organization are working to execute proper scheduled vaccination and execute the government vaccination programme like Mission Indradhanush, Pulse Polio, Measales Rubella Vaccination etc under the Direct supervision and Guidance of World Health organization Guwahati unit in the district of Nalbari,Kamrup(M),Darrang and Baska District since 2017.
